The Aboriginal Health and Medical Research Council has created a culturally appropriate toolkit to guide Aboriginal people through end-of-life care. The ‘Journey to Dreaming Diary’ incorporates storytelling, artwork, mindfulness and practical activities to prepare Aboriginal patients for their end-of-life journey.
