Awabakal has provided health & community services to Aboriginal people living across the Newcastle and surrounding regions for more than 40 years and is dedicated to advancing the wellbeing of our community.

About the role

We are looking for an organised and motivated individual to provide coordination of the delivery of culturally safe social and emotional wellbeing services and support for Awabakal Clients, by working closely with and guiding the practice of community network partners and Awabakal staff. Duties include but not limited to:

  • Ensure the client has their psychosocial health needs met in accordance with their mental health care plan
  • Provide timely case management to clients, involving assessment, care planning, care coordination of service delivery and ongoing assessment of client needs
  • Participate in clinical case conferencing with Awabakal mental health team to ensue coordinated and integrated management
  • Assist in the provision of health promotion, referrals, advice, and information as part of social support
  • Work collaboratively with Awabakal primary and secondary allied health teams to deliver appropriate services
  • Provide support through advocacy with internal and external providers
  • Support the client’s safety plan and facilitate self-management strategies in times of crisis
  • Undertake assertive outreach to ensure hard to reach populations are engaged in the service
  • Facilitate and deliver culturally appropriate therapeutic groups

About you

  • Cert III Aboriginal Health Worker/Practitioner or working towards
  • NSW Drivers License
  • First Aid and CPR Certificate
  • National Police Check
  • Working with Children Check

Prior to an offer of employment being made, candidates will be required to undertake relevant background checks including a Criminal History and Working with Children Check and demonstrate evidence of current registration, and original qualifications.
